1. Cashless Request for Pre-Authorisation
Complete the pre-authorisation form available at the hospital's insurance/TPA Desk and send us through fax.
I. Approval : Approval letter sent by the claim management team
II. Query : Hospital/lnsured to response to the query raised by the claim management team
III. Rejected : You may initiate the treatment and file for re-imbursement claim
2. Re-imbursement Claim Form Submission
Submission of claim form along with required documents, as per the policy terms & conditions.
I. Approval : Approval letter sent by the claim management team
Query : Insured to respond to the query raised by the claim management team
Rejected ; We will communicate the reason in case of rejection
